These preventive services help identify health concerns early — often before symptoms begin — when they are easier to treat or even prevent. Below is a quick guide explaining what these services are and why they are important for you and your ʻohana’s health.

Preventive Screenings

 

Colorectal Cancer Screening

Eligibility:

  • Adults 45–75

Frequency:

  • Stool test (FIT): every year
  • Stool DNA test: every 3 years
  • Colonoscopy: every 10 years

What you do:

  • Ask your provider about screening options
  • Complete an at-home stool test or schedule a colonoscopy

Why it matters:

  • Can prevent cancer before it starts
  • Finds cancer early when it’s easier to treat

Breast Cancer Screening

Eligibility:

  • Women ages 40–74

Frequency:

  • Every 2 years

What you do:

  • Talk with your provider about getting a mammogram, you will need an order from your provider for a radiology appointment.
  • Schedule your appointment with a radiology center

Why it matters:

  • Finds cancer early, when it’s easier to treat
  • Early detection saves lives

Cervical Cancer Screening (HPV/Pap)

Eligibility:

  • Women ages 21–64

Frequency:

  • Ages 21–29: Pap test every 3 years
  • Ages 30–64: HPV test every 5 years (preferred)

What you do:

  • Complete an HPV self-swab during your visit
  • Or schedule a Pap test with your provider

Why it matters:

  • Can prevent cancer before it starts
  • Finds abnormal cells early so they can be treated

Adult Visits

Annual Wellness Visit

Eligibility:

  • Adults ages 18 and older
  • Medicare patients are eligible for a Medicare Annual Wellness Visit after having Medicare Part B for more than 12 months

Frequency:

  • Once a year

What you do:

  • Schedule and attend your yearly wellness visit
  • Review your health history, medications, screenings, and wellness goals with your provider

Why it matters:

  • Helps prevent illness and identify health concerns early
  • Keeps you up to date on recommended screenings, vaccines, and preventive care
  • Supports your overall health and well-being

Blood Pressure Control (High Blood Pressure)

Eligibility:

  • Adults 18–85 with high blood pressure

Frequency:

  • Every visit
  • Patients diagnosed with High Blood Pressure may be called in for special appointments to monitor your BP

What you do:

  • Have your blood pressure checked at each visit
  • Follow your care plan and medications if prescribed

Why it matters:

  • Helps prevent stroke, heart attack, and kidney disease

Diabetes (A1c Control)

Eligibility:

  • Adults 18–75 with diabetes

Frequency:

  • 1–2 times per year

What you do:

  • Get your A1c blood test as recommended
  • Follow up with your provider to review results

Why it matters:

  • Helps prevent serious complications like vision loss and kidney disease

Prenatal Care – First Visit

Eligibility:

  • Pregnant patients

Frequency:

  • First visit in the first trimester (or as soon as possible)

What you do:

  • Schedule your first prenatal visit as soon as you know you’re pregnant

  • Early care helps ensure a healthy pregnancy and baby

  • Patients who recently had a baby

Frequency:

  • Visit between 1–12 weeks after delivery

What you do:

  • Schedule your postpartum visit before leaving the hospital or during pregnancy
  • Attend your follow-up visit
